TROOP 650 PERMISSION SLIP FOR SCOUT YEAR 2008-2009

My daughter,                                               has permission to participate in the events that I have checked if our family schedule permits.  I understand that I am responsible for contacting the leader if my daughter is not able to attend an even.  I give my permission for my daughter to be transported to the scout lodge on Auburn Road during the school year of 2008-2009.  I understand that if I am more than 30 minutes late, Denise Sutter has my approval to take my daughter to her home where I will make arrangements to come and pick her up.

 

The following applies to my daughter’s blanket health condition that applies to all events listed below.  I will contact my daughters leader if anything changes that would impact my daughter at the event.
